ViaQuest Healthcare Central offers facility-based services in three, five-bed residential homes located in the community. We provide quality and well-trained staff to provide multiple services based on the individual’s wants and needs 24-hours a day. ViaQuest Healthcare Central ensures the individual’s health and welfare through assisting with, and providing teaching in the areas of
- Hygiene
- Nutritional services
- Medication administration and health management
- Community access and socialization
- Behavior support
- Financial management
Active Treatment Services
ViaQuest Healthcare Central offers 24-hour staffing and nursing to ensure that each individual participates in active treatment and is in compliance with Medicaid regulation section 483.440(d) (2) in part or in total. Development of the daily active treatment schedule is as follows:
- After the individual’s initial 30-day interdisciplinary review, the QMRP/Administrator will develop the daily schedule.
- The daily schedule will be reviewed and updated at each individual’s annual team meeting and as needed.
- The Home Manager or designee will investigate any pattern of staff action, or scheduling that results in all or a majority of the individual’s routinely engaging in the same activity or routine at the same time.

Additional services are available, including occupational therapy, speech therapy, physical therapy, recreational therapy, counseling and psychological services, medical/nursing and continuous active treatment services.
We also link to Mental Health Services through our related company, ViaQuest Behavioral Health (VBH). These sessions are offered on an individual basis or in a group setting, and are individualized based on each individual’s need. The group sessions are formatted with themes beneficial to all participants such as life skills, diversity, work relationships, anger management, and more. Individuals must be Medicaid eligible to receive these services. Guardian approval is required and individual consideration is taken in to account to determine needs, services and development of curriculum.
Active Treatment Schedule Guidelines
The daily schedule will:
- Be individualized and consistent with the individual’s objectives
- Direct the intensity of daily activity for individuals living at the facility
- Provide for a range of options, rather than a fixed regimen (to the extent possible)
- Allow the individual an opportunity to choose individual or group activities and to engage in them as independently and freely as possible (staff schedules and routines should be supportive of this goal and result in reasonable choices for individuals)
- Reflect normal daily routine
- Reflect the individual’s training programs, including education
- Be trained to, and known by, all staff working with the individual
- Be implemented daily (an alternate plan must be developed to accommodate changes in the individual’s typical routine; i.e. extended absences from workshop or educational program, extended illness, vacation)
- Be readily available to relevant staff
