Health Service Coordination

Health Service Coordination is a specialized type of service coordination intended to provide individuals with professional nursing services to coordinate and oversee their medical care and needs. Our goal is that Health Service Coordination will ensure that individuals are healthy and safe in their residential setting. Our focus is not only management of current medical conditions, but prevention of future ones while supplying on site support. HSC is individualized, taking into consideration each person’s needs and desires.
